I give Grace Baptist Church Designated Staff Member permission to administer any first aid (ice packs, medication for stings or scrapes, band-aids, bandages) or seek out emergency medical treatment if such treatment may be necessary. I also agree that I will not hold Grace Baptist Church liable, or any property owners of which Grace Baptist Church is located, or any person or persons associated with Grace Baptist Church’s operation liable for any injuries that may be incurred while at Grace Baptist Church. I also grant permission for my camper to be pictured and that such pictures may be used for future promotions, brochures or web design.
